• Care Management: links frail older persons with community services, includes an in-home assessment by a registered nurse & social worker, followed by arrangements for service delivery.
• Case Coordination & Support: accesses the eligibility of seniors for in-home services such as Personal Care, Homemaker and Respite Care. After determination of eligibility, service provision is arranged.

2. Can I join a senior citizen club if I am not a resident?  No

4. Do you provide transportation?  No, however transportation is provided by SMART by calling (866) 962-5515. Note: a minimum 24-hour reservation is required.
5. Do you serve free meals at your Center?  Congregate meals are served at both the Eton and Berwyn Senior Center for a donation.
6. Does your Center provide blood pressure checks?  Yes, both the Eton and Berwyn Center do.
